STEP 1: ELIGIBILITY

Children are eligible for equipment based on the following criteria:

17 years of age or under
Requests equipment that qualifies as a valid health need
Lives in the Greater St. Louis Region
Referred by a social worker, case manager, etc.
Requests up to $1000 for Adaptive Equipment

STEP 2: APPLICATION

STEP 3: DOCUMENTS

To fully evaluate the application, email or mail the following required documents to The Arya Foundation: (Do NOT fax the documents)

Letter from the physician stating the necessity of the equipment
Invoice from the provider or online URL
Child’s Photo & Story
Our team will confirm your referral.

STEP 4: REVIEW

Within 30 days of the application being submitted and the required documents being received, an application decision will be sent by email. If approved:

Based on the equipment, it will be shipped to the child, or a check made payable to the provider will be sent to the family.
